In some cases we can use intramuscular magnesium and B vitamins for fatigue if the IVs are impractical, though we have limits on what can be injected compared to the IV Like the IV therapy, IM shots increase tissue levels of nutrients more efficiently than oral supplementation.     Chronic Fatigue Syndrome Doctors Report

    Dr. Bell noted that a year of daily 1 liter saline infusions had enabled a severely ill CFS patient who had been undergoing seizures to return to full-time work.

    With the first bag of saline the pseudo-seizures stopped and did not return. At three months of daily infusions of 1 liter of normal saline, she was able to be up and around the house for several hours a day.     Subject Compliance To The Clinical Trial

    Of the 150 subjects initially recruited for the trial, 147 were enrolled in the final study group. Four participants dropped out of the study the day after intervention. The dropped participants completed two outcome measurements , but did not complete the third measurement the next day. Thus, we initially analyzed data from the remaining 143 participants. Two outliers for plasma vitamin C level were excluded from analysis. Our final data set, then, included 141 participants . The baseline characteristics of participants are shown in Table . There were no demographical differences between two groups. Lifestyles can influence fatigue however, it did not differ between the two groups.

    While many amino acids are necessary in the therapy of FMS/CFS in the authors clinical experience tow amino structures found clinically to be exceptionally helpful are taurine and carnitine.

    Carnitine is useful in varied targets affecting the FMS/CFS patient including decreasing neurotoxicity , decreasing lactic acid build up as well as its more commonly known biochemical function of transporting fatty acids into the mitochondria for beta oxidation based energy production. The l-carnitine form in our clinic is administered IB at doses of 500 4000 mg and the acetyl-l-carnitine form at doses of 100 mg to 1000 mg in most cases.

    Taurine is the master osmolyte in the human body and as such regulates distribution of the excitable ions to their appropriate sides of the cell membrane . In this role the authors clinical observation has been that the addition of taurine to IV formulas containing magnesium and other excitable tissue acting minerals causes a greater benefit as reported by patients. For example the addition of taurine to a formula with magnesium will be perceived by patients to have a more muscle relaxing effect in many cases. Taurine is used constantly at the cell membrane and thus depleted both in low dietary intake as well as by oxidative stressors .     Chronic Fatigue Syndrome Fibromyalgia And Small Fiber Neuropathy Studies

    Significant interest in gamma globulin resulted in several clinical trials in the 1990s which had mixed results. Hickie et. al found improved mood and immune functioning in a smaller 1992 IVIG trial and Lloyds placebo-controlled 1990 trial found significant improvement in approximately 40% of patients with many resuming work and other activities. Lloyds larger followup trial, however, was unsuccessful, as was a smaller American trial that targeted IgG deficient individuals.

    Rowes 1997 IVIG trial, on the other hand, found significant functional improvement as did her five year followup. Rowes five year followup reported in a publication Journal for Chronic Fatigue Syndrome that was not included in medical indexes was not unfortunately widely disseminated.

    More recently, gamma globulin therapy was successful with several parvovirus-B19 associated cases of chronic fatigue syndrome.

    Fibromyalgia and Small Fiber Polyneuropathy

    Fibromyalgia patients with evidence of nerve demyelination caused by immune dysfunction responded positively to a short-term IVIG trial.

    Dr. Oaklander reported promising results using IVIG in children with a putative diagnosis of small-fiber polyneuropathy e.g. small fiber neuropathy . SFPN or SFN has been documented in fibromyalgia and is currently being documented in chronic fatigue syndrome . . A small fiber neuropathy diagnosis could help patients get IVIG covered by insurance companies.

    The use of ASC in IV therapy is well known and reasonably well studied and reported on elsewhere in the literature and clinical experience.
