Fatigue is a symptom that usually has some underlying cause. Other symptoms such as fainting or loss of consciousness , near-syncope, rapid heartbeat , dizziness or vertigo may also be described as part of the fatigue experienced by the affected individual. The presence of these symptoms may actually help lead a health care practitioner to discover the underlying cause of the fatigue.
The potential causes of fatigue are numerous. The majority of diseases known to man often list fatigue or malaise as possible associated symptoms. This is complicated by the fact that fatigue can occur in normal healthy individuals as a normal response to physical and mental exertion. However, normal fatigue may begin to become abnormal if it becomes chronic, extreme or prolonged fatigue usually this occurs when a person experiences chronic or prolonged physical or mental exertion. For example, unusually hard physical or mental exertion for one day can result in normal fatigue that may last about a day or sometimes more, depending on the exertion level, while daily unusually hard physical or mental exertion may result in prolonged fatigue . This latter situation may develop into abnormal fatigue.

People with ME/CFS often have their illness begin in a way that reminds them of getting the flu. This has made researchers suspect an infection may trigger ME/CFS. - Stop the physical activity well before you feel any symptom flare. Pacing yourself is very important.

How Is A Pots Diagnosis Made
Problems with the autonomic nervous system can be difficult to diagnose. The standard test for POTS is the tilt table test. During the test, you are secured to a table that tilts from being horizontal to almost a 90-degree angle. The test measures your hearts response to switching from laying down to standing up. Some people with POTS faint during this test, even if they rarely faint standing up.
Although the test seems straightforward, many things can interfere with it. Its important that a POTS specialist oversees it. Its best to work with a doctor who has diagnosed and treated patients with POTS. This could be a cardiologist, a neuromuscular specialist or another doctor.
A Gradual Onset Preceded By An Infectious Event Is The Most Common Pattern
The most common onset pattern was a distinct change in health heralded by an infectious event followed by a gradual progression to becoming consistently sick. Despite offering 14 possible precipitating factors and an open text box, almost two-thirds of our subjects selected only one or two factors. The top three factors were an infectious illness , stress or a major life event , and exposure to an environmental/ chemical toxin . Stressful events were rarely chosen as the only precipitant though, endorsed only by 8% of our subjects, and appeared mostly in conjunction with infection or other precipitants. These results agree with prior studies: 4993% of subjects reported an infection-like illness while 4395% noted significant stress in the months or years preceding or surrounding the beginning of their illness . Becker also found that 99% of their subjects chose only 1 or 2 factors and both he and Evans showed < 15% of subjects endorsed stress as the sole precipitant.

It reduces the likelihood of triggering PEM and can help reduce symptoms. Pacing is not a cure for ME/CFS and does not treat the underlying, physical cause of the disease.
The goal of pacing is for activity to be sustainable, so that the person can have some stability in their symptoms and what they can do each day. Pacing should be used in conjunction with rest.
The most challenging aspect of pacing is that PEM is very often delayed by as much as 24-48 hours, which can make it difficult to recognise when youve done too much. Using tools like an activity diary or heart rate monitor help you learn about your body and how much activity you can safely tolerate.

Fatigue Caused By An Unhealthy Diet
The old adage you are what you eat is really true. By eating well-balanced meals and snacks that contain enough calories and nutrients, you give yourself sufficient energy to counteract extreme tiredness. Simply eating to stave off hunger during the day is not sufficient sugary and processed foods are counterproductive, and rob us of essential nutrients needed for energy production. Too many refined carbohydrates, such as bread, cause our blood sugar levels to spike and crash, leaving us exhausted. Make sure that your diet is made up of plenty of nutrient-rich foods such as whole grains, fresh fruit and vegetables.
Your body can function within a range of calories depending on your weight, height, age, and other factors. However, generally speaking, women should consume about 2,000 calories a day, and men 2,500 calories a day. Consuming less than 1,200 calories per day may cause your metabolism to slow down.
Conversely, if you eat too much, this can also cause fatigue. The key to healthy eating is therefore to choose certain foods that contain adequate nutrients and make sure you have meals and snacks in appropriate portions.

Family Medical History: First Degree Relatives
Thirteen percent recounted at least one first-degree relative who was also diagnosed with ME/CFS and 21%, at least one FDR affected by chronic fatigue of unclear etiology. In total, 27% of subjects came from families with FDRs affected by ME/CFS or chronic fatigue of unclear etiology. Thirty-five percent of subjects also described at least one FDR afflicted by an autoimmune disorder.

Shift Work Sleep Disorder
Shift work can wreak havoc on your body’s 24-hour internal clock, or circadian rhythm. When you work nights or rotate shifts, your body doesn’t know when to be awake and when to sleep, which causes fatigue.
Daylight is often a cue to be awake. If you must sleep during the day, try to make your sleeping area as dark, cool, and quiet as possible. If you must work at night, keep your workplace brightly lit. Try to work night shifts all in a row and avoid frequently rotating shifts. Stay away from caffeine, and stick to a regular sleep-wake schedule as much as possible on days off.

Chronic fatigue syndrome , also termed myalgic encephalomyelitis is a disorder characterized by chronic fatigue. The disorder has no apparent cause.
CFS has also been named systemic exertion intolerance disease to better reflect the conditions hallmark which is post-exertional malaise. Post-exertional malaise describes a massive energy crash after a relatively minor exertion.
CSF is a heterogenous disorder. However, certain features are common to nearly all affected patients
Key clinical features include :
- Onset that may be sudden, often associated with a typical infection such as an upper respiratory infection or mononucleosis, or gradual over several months.
- Overwhelming fatigue associated with additional symptoms .
- Symptoms characteristically exacerbated by excessive physical activity.
- A pre-CFS medical history that is not one of multiple somatic problems . Affected patients are typically highly functioning individuals who are struck down with this disease.
The cause of CFS is unknown, but it is believed to be an infectious disease with immunologic manifestations. There is often a history of an antecedent infection that precipitated the prolonged state of fatigue following the initial illness. Some have suggested that the infectious agent responsible for CFS is Chlamydia pneumoniae.
Recent evidence suggest that Covid-19 patients are at risk of developing CFS .
It is estimated that between 836.000 and 2.5 million individuals are affected by CFS in the United States .
The Acute Stage Of Me/cfs
In the early, acute, febrile stage of ME/CFS, the diagnosis can be uncertain and other causes of fever need to be considered. Adequate rest and activity management are the mainstays of treatment. Premature resumption of activity or attempts to return to school can result in a relapse or increased severity of symptoms.

Further Exploration Of Other Potential Triggers Is Needed
Twenty percent of our subjects noted that an exposure to a chemical or environmental toxin might have a played an initiating role in their illness. Two independent Australian research groups published similar results: in Clark et al. , 16% endorsed exposure to environmental toxins while in Johnston et al. 6% reported mold 11%,toxic chemicals 6%, poor recycled air and 4%, heavy metals . In contrast, in Friedberg’s US-based study, 44% of subjects perceived toxic exposure to be a source of their illness . It is unclear why Friedberg’s study yielded double the percentages we and the Australians found. None of these researchers commented further on these findings in their articles.
Since this survey was constructed as a broad overview, we did not take a comprehensive history of possible exposures. Additionally, because the published literature in this area is sparse and composed of mostly case studies , it is difficult to know what specific substances to concentrate on. Written-in responses from our subjects may provide leads but were too imprecise and disparate to draw any solid conclusions. Subjects’ answers might also be influenced by, for example, recall bias, misattribution, other patients’ accounts, and media outlets.
