How Is Chronic Fatigue Syndrome Diagnosed

CFS diagnosis depends on two criteria:

  • Severity and duration. The severe and chronic tiredness lasts for more than 6 months and other medical conditions have been ruled out.
  • Number of symptoms. Four or more symptoms of CFS are present.
  • A specific treatment for CFS has yet to be proven effective. Vitamin supplements and medicines have some benefit. Many treatments just relieve the symptoms of CFS.

    What Test Is Like

    There is no specific test to diagnose CFS, so it can be challenging for doctors. Doctors usually use the functional medicine approach, where they look at the symptoms and test to see how well your body is functioning. This approach also helps them understand whats causing the problem so that therapies can be targeted specifically for them.

    Some of the common tests done include saliva or blood testing to check on adrenal fatigue, food sensitivities testing, endocrine health tests, external stressors, and more.

    This testing looks at the levels of cortisol and DHEA in your body, which can give a better understanding of how your adrenals are functioning. If theyre low, it might show that you have fatigue or tiredness because your adrenals arent doing their job effectively to produce energy.

    Low blood sugar may also be an indicator if you tend to feel fatigued after meals or experience brain fog due to hypoglycemia . These symptoms definitely indicate that theres some stress going on with the endocrine system.

    Chronic Fatigue Syndrome Test Through A Haematologist

    Your doctor can and should take blood tests for you.

    Early blood tests showed that I had an ongoing viral infection at a time when I no longer had clear symptoms of a virus. These test results were consistent with the theory of a viral cause for CFS.

    Two consecutive tests showed a low white blood cell count and I was referred to a haematologist fairly early on in the illness.

    Please note that you may not have any positive result from a blood test. Many people with CFS do not. It does NOT mean you are not ill.

    I was lucky to be sent to a haematologist who was an inquiring doctor and took my symptoms seriously even though I had no diagnosis at the time.

    In 1993 after 18 months of serious illness, I was finally diagnosed with ME . The haematologist then said he had thought that was the illness I had from the blood test results.

    I have always pondered on the implication that the blood test results were in some way able to be used as a ME / Chronic Fatigue Syndrome test by someone with sufficient knowledge.

    It reassured me to visit the haemotologist every few years to have blood tests repeated.

    Someone with chronic fatigue syndrome can have many possible symptoms. The most common ones include:

    • severe fatigue, which can make it hard to get out of bed and do normal daily activities
    • sleep problems, such as trouble falling or staying asleep, or not having a refreshing sleep
    • symptoms getting worse after physical or mental effort
    • symptoms or dizziness that get worse after standing up or sitting upright from a lying down position
    • problems with concentration and memory
    • headaches and stomachaches

    What Causes Chronic Fatigue Syndrome

    The cause of CFS is not yet fully understood, but it is likely that several factors contribute to its development. There are changes in the immune system and body chemistry.

    CFS is usually triggered by a viral infection, such as glandular fever or influenza, but any infection may be the trigger. It is likely that the potential for getting CFS is genetic and runs in families. Some people experience ongoing effects of COVID-19 infection, known as long COVID, which are very similar to the symptoms of chronic fatigue syndrome.

    Other factors that may contribute include:

    • emotional or physical distress
    • multiple joint pain without joint swelling or redness
    • headaches of a new type, pattern or severity
    • unrefreshing sleep
    • tiredness that can last for more than 24 hours after physical or mental effort.

    Some people also report a range of other symptoms such as nausea, loss of appetite, irritable bowel syndrome , bloating, diarrhoea, irregular heartbeat, chest pain, jaw pain, night sweats, increased sensitivity to alcohol or medication, chronic cough, dizzy spells or dry eyes.

    The severity of symptoms can vary from day to day, or even within a day.

    Diagnostic Criteria For Chronic Fatigue Syndrome

    Your doctor will order tests only when you meet diagnostic criteria. It means you should have persistent, unexplained fatigue for at least six months with other symptoms. Here is more about these symptoms:

    Fatigue

    Persistent tiredness or fatigue is the most common symptom of Chronic Fatigue Syndrome. The extreme exhaustion lasts for more than 24 hours after mental or physical exercise and keeps you from living comfortably. The tiredness you experience in CFS will not go away with rest, is different from everyday tiredness, is not due to muscle weakness, is not like tiredness after over-exertion, and is not accompanied by loss of pleasure or motivation.

    Other Symptoms

    In addition to fatigue, there will be some other symptoms as well.

    • You will experience cognitive difficulties, such as bad short-term memory, poor concentration and reduced attention span. You may also find it difficult to organize your thoughts and feel disorientated.
    • You will experience sleeping difficulties. Some people are unable to sleep while others may face issues such as early waking or disrupted sleep/wake patterns.
    • You will notice muscular pains, headaches and joint pains as well.

    Some people with CFS have also reported some other symptoms, such as palpitations, nausea and dizziness.

    Severity of the Symptoms

    • Mild

    If your symptoms are mild, you will be able to do light domestic tasks with some difficulty. Still, you will be able to complete your jobs.

    • Moderate
    • Severe

    Scientists have developed a test for chronic fatigue syndrome that detects the reaction of the immune cells and blood plasma to stress. The findings may also help screen effective drugs for the condition.

    Currently, physicians can only diagnose ME/CFS by examining a persons symptoms and medical history, and by excluding other possible illnesses.

    This can make the diagnosis process difficult, lengthy, and inaccurate.

    However, the results of a new study may soon change this. Scientists at Stanford University School of Medicine in California have discovered a biomarker for ME/CFS and developed a test that could soon diagnose the condition accurately.

    Serum Iron And Ferritin

    Serum iron:

    Serum iron measures the amount of iron in transit through your blood. If youve just eaten a big serving of beef liver, youre likely to have an increase in serum iron. This is why testing only serum iron is not sufficient to determine whether you have sufficient iron stores.

    Serum iron should always be tested with ferritin and a CBC .

    Conventional reference range: 40155 µg/dL

    Functional reference range: 40135 µg/dL

    Both high and low levels of iron can be a cause of fatigue. If either value is out of range, encourage your doctor to explore further testing.

    Ferritin:

    Ferritin is the storage protein for iron. Ferritin stores and slowly releases iron in a non-toxic and controlled way, which helps protect against iron deficiency and iron overload. Ferritin can also be utilized by your body to protect it against oxidative stress and inflammation. A wide variety of acute and chronic inflammatory conditions involve the production of cells/proteins that increase ferritin levels.

    If youre not dealing with CFS, serum ferritin is an excellent indirect marker of the total amount of iron stored in your body. This is no longer true in chronic fatigue syndrome, as ferritin plays the dual roles of iron storage protein and acute-phase reactant. If you have CFS, you cannot rely solely on ferritin as a marker for iron levels. It is likely to be elevated to help your body better manage inflammation.

    Chronic Fatigue Syndrome And Similar Disorders

    Chronic fatigue syndrome encompasses a wide range of symptoms that affect most systems in the body, so doctors may easily confuse it for another disorder. Examples of the many conditions that may cause similar symptoms include anemia, hypothyroidism, hyperthyroidism, diabetes, cancer, celiac disease, obstructive sleep apnea, and inflammatory bowel disease.

    Chronic fatigue syndrome may look especially similar to hypersomnia, narcolepsy, fibromyalgia, depression, and obstructive sleep apnea.

    There is no single test available to help make correct diagnosis for CFS. But a self-assessment is available here. In the absence of a reliable test for chronic fatigue syndrome, the doctor has to rule out other conditions to determine if CFS is the leading problem in your case. Your doctor may order laboratory and imaging tests to get to a conclusion.

    Laboratory Tests

    Your doctor asks for laboratory tests to ensure you do not have other fatigue-causing diseases. They usually order the following tests:

    • Adrenal, thyroid and liver function tests
    • Erythrocyte sedimentation rate which helps identify if there is inflammation in your body
    • Antibody tests which help determine if you have had any infection in the past, such as Chlamydia or Lyme disease

    Imaging or Scans

    Along with ordering a chronic fatigue syndrome test, your doctor may also perform the following imaging studies.

    • They may order an MRI or CT scans to ensure you do not have any disorder of the central nervous system. If you only have CFS, your MRIs and CT scans will be normal.
    • They may also order positron emission tomography and single-photon emission computed tomography scans to see if your brain is getting enough blood. Restricted blood flow may lead to short-term memory problems which are common in chronic fatigue syndrome.
