ME/CFS can affect people of all ages, including the very young. Young people with ME/CFS, especially children under the age of ten, might have trouble knowing or articulating that something is wrong. They may not realise that their loss of stamina, or other symptoms they are experiencing, are not normal. Alternatively, they may not have the words to describe what they are feeling.

For both younger children and teenagers, schooling can be affected but ME/CFS is not school refusal. While school refusal can often be accompanied by physical complaints , and these complaints may tend to disappear when the young person stays home, the pattern is different with ME/CFS. In a young person with ME/CFS, some symptoms never ease and symptom exacerbation is triggered by activity. Physiologically, the essential amino acid tryptophan is metabolized approximately 5% to serotonin and melatonin, and degraded approximately 95% by the enzyme IDO to kynurenine . IDO and the kynurenine pathway play important roles in the immune system as immunoregulators and immunosuppressors . IDO is induced by infection and inflammation through pro-inflammatory cytokines and mediators, mainly by IFN-y, but also IFN-, TNF- and LPS . Elevated IDO expressions seem to be an important hallmark of chronic viral infections such as HIV, hepatitis B and C by building an immune tolerant environment. Although elevated levels of IDO seem to correlate with the severity of diseases, the relevance of these findings remains unestablished . IDO activates regulatory T-cells and suppresses effector T-cell proliferation and function by the degradation of tryptophan to kynurenine , which enhances immunologic tolerance towards chronic infections.

Increased Gut Permeability And Bacterial Translocation In Me/cfs Patients

Another reason for increased intestinal permeability might come from the increased by-products of the amino acid fermentation to produce SCFA , as discussed below.

Bacterial translocation could also be important concerning PEM. Shukla and co-workers examined this connection in a small study by observing phenotype characteristics, post-exertion malaise and microbiome changes 15 minutes, 48 and 72 hours after a maximal exercise test. Besides the expected large changes in pain, fatigue and confusion, interestingly also the relative abundance of the intestinal microbiota changed differentially than in the control group. Following exercise, the genus Clostridium was increased in the blood fifteen minutes later and abundances of Bacilli were significantly higher 48 hours later, which might be due to increased post-exertional bacterial translocation. The authors also pointed out that researchers should rather focus on the post-exertional changes in intestinal composition, than on the dysbiosis itself .

In health, epithelial cells are tightly connected by desmosomes, adherens junctions and tight junctions, which are made up of occludin, claudins, and junctional adhesion molecules respectively. Thus, intraluminal translocation of bacteria and toxins into the bloodstream is prevented . However, when homeostasis is altered, for example due to gut inflammation, dysbiosis, chronic NSAID intake, or stress, the barrier integrity is lost and commensal bacteria can reach the bloodstream . The presence of circulating lipopolysaccharide derived from gram-negative endobacteria, also known as metabolic endotoxemia, then activates the inflammatory TLR4 pathway and immune cells produce pro-inflammatory cytokines and LPS-directed IgM/IgA, thus enhancing systemic inflammation .

Intestinal bacteria yield clues to the mysterious ailment, researchers say

THURSDAY, April 27, 2017 — Scientists have found differences in the gut bacteria of people with chronic fatigue syndrome versus their healthy peers.

The finding is among the first to link abnormalities in the makeup of gut bacteria — the “microbiome” — and chronic fatigue, a mysterious and debilitating malady.

Whether these differences are merely a sign of chronic fatigue syndrome or an underlying cause isn’t clear, said study lead author Dr. W. Ian Lipkin.

But they could be tied to disease severity, said Lipkin. He is director of the Center for Infection and Immunity at Columbia University Mailman School of Public Health.

Chronic fatigue syndrome affects about 1 million Americans — women more often than men, according to the U.S. Centers for Disease Control and Prevention.

People with the syndrome typically complain of extreme fatigue after exertion, muscle and joint pain, thinking difficulty and sleep problems. But only about 20 percent of people who have the syndrome actually know it, because it’s difficult to diagnose, the CDC notes.

Scientists have begun to look to the microbiome for answers to a host of medical mysteries.

Sequencing Data Analyses Of Intestinal And Oral Microbiota

Illumina sequencing data were pre-processed for the OTU- and taxonomy-based analyses. After the assembly and demultiplexing of the Illumina paired-end sequences, a total of 9,967,562 and 4,774,706 raw sequences was obtained for fecal and salivary samples respectively. After quality trimming and chimera checking, these were reduced to 8,788,426 and 3,508,491 filtered sequences. To avoid biases in diversity estimates due to differences in the number of sequences per sample, rarefaction to a common minimum number of 6100 and 5200 sequences per sample was performed for fecal and salivary samples respectively, which represent the number of sequences of the lowest populated sample. After the rarefaction step, 481,803 high quality sequences for fecal samples and 585,579 high-quality sequences for salivary samples remained for further analyses. A high average Goods coverage was obtained for fecal samples, highlighting that most of bacterial diversity in the samples was represented. Conversely, in salivary samples the Goods coverage index was lower .

Subsequently, the reads were clustered in OTUs at 97% similarity, without any sub-filtering for rare OTUs, and resulted in a total of 4231 OTUs for fecal samples and 78,000 OTUs for salivary samples, with an average number of 207 and 1235 OTUs per sample respectively.
