Abstract:

Certain therapeutic microbes, including Bifidobacteria infantis 35624 exert beneficial immunoregulatory effects by mimicking commensal-immune interactions however, the value of these effects in patients with non-gastrointestinal inflammatory conditions remains unclear.

In this study, we assessed the impact of oral administration of B. infantis 35624, for 68 weeks on inflammatory biomarker and plasma cytokine levels in patients with ulcerative colitis , chronic fatigue syndrome and psoriasis in three separate randomized, double-blind, placebo-controlled interventions. Additionally, the effect of B. infantis 35624 on immunological biomarkers in healthy subjects was assessed.

At baseline, both gastrointestinal and non-gastrointestinal patients had significantly increased plasma levels of C-reactive protein and the pro-inflammatory cytokines tumor necrosis factor and interleukin-6 compared with healthy volunteers. B. infantis 35624 feeding resulted in reduced plasma CRP levels in all three inflammatory disorders compared with placebo.

Interestingly, plasma TNF- was reduced in CFS and psoriasis while IL-6 was reduced in UC and CFS. Furthermore, in healthy subjects, LPS-stimulated TNF- and IL-6 secretion by peripheral blood mononuclear cells was significantly reduced in the B. infantis 35624-treated groups compared with placebo following eight weeks of feeding.

    1. Introduction

    Patients with CFS/ME exhibit an abnormal immune response to exercise, oxidative stress system dysfunction, and modification in the gene expression profile of immune cells . The increased expression of toll-like receptor 4 correlated with postexertional malaise, which is the main feature of ME/CFS, suggests the existence of an impaired interaction between these immune receptors and pathogenic or xenobiotic agents .

    2. Materials and Methods

    2.1. Study Population

    Patients with CFS/ME diagnosis according to Fukudas criteria referred to the clinic of Internal Medicine and Geriatrics ASP-IDR Santa Margherita who gave their consent to participate in the trial were enrolled for the study. The exclusion criteria were as follows: lack of consent, overlap with other diseases, early diagnosis of psychiatric illness, somatoform disorders, abuse of alcohol and drugs, antibiotic therapy, and presence of risk factors for probiotic sepsis according to Boyles criteria .

    It is important to note that correlation of a specific psychiatric illness with species of gut bacteria does not necessarily mean that the abnormal gut caused the condition. As was already mentioned, the relationship between the gut and the brain is multidirectional. While pathogenic gut bacteria could certainly be a direct, standalone cause of certain psychiatric disorders among a subset of individuals it may have not been a causative factor for others.

    For some individuals, it may be that gut dysbiosis is a causative factor in the pathogenesis of a particular condition, but it may only contribute to a small extent. For example, someone with schizophrenia may have developed the condition in part because of pathogenic bacteria in the gut, but these bacteria may have only played tiny role in the disease onset. Therefore, it is important to avoid assuming that simply fixing the gut is some utopian treatment or preventative intervention.

    To briefly summarize, gut bacteria may be: more of a symptom or byproduct of a condition , a 100% direct cause, or a partial cause of varying weightedness depending on the individual. Consider that all three aforestated possibilities may occur rather than assuming one is the absolute, definitive explanation. That said, regardless of whether the chicken or egg came first, evidence does suggest that fixing the gut may improve neurophysiological function.
